Copy of student's birth certificate and copy of passport or identification card (if any); copy of household registration; 3 photos of student (3x4 cm, white or blue background); 2 photos of each parent (3x4 cm); copies of both parents' passports or identification cards; previous school records (official transcript) and recommendation letters; health records (signed by a certified physician) and vaccination certificate
